American Family Children’s Hospital Pediatric Isolation Precautions Parent/Guardian Agreement Hand hygiene and covering your cough is the best way to prevent the spread of infection. Isolation precautions are extra measures that are sometimes put into place to lessen the spread of infections within the hospital. When strictly followed, these safety measures can help keep patients, visitors, and health care workers safe from infection. The safety of our patients, visitors, and staff is up to all of us. ____ I know that my child is in precautions (circle) Droplet or Contact or Enhanced Contact. My child is assumed or confirmed to have an infection or carry a germ that may cause an infection. Droplet Precautions helps stop the spread of germs that can be spread by secretions from the nose or mouth. Contact and Enhanced Contact Precautions help stop the spread of germs that can be spread by touching those who carry the germ on their skin or by touching items that have the germ on it. ____ I have received the Health Facts for You titled, “Isolation Precautions for Pediatric Patients”. I understand the detailed safety measures that should be taken to avoid the spread of infection in the hospital. These include hand hygiene and protective clothing. ____ I know that although these strict precautions are preferred, I may opt out of wearing protective clothing while spending time with my child. If I opt out, to help lessen the spread of infection to other children and families, I must follow strict hand hygiene when I enter and leave my child’s room. Hand hygiene should be done even if I decide to wear protective clothing. ____ If I opt out of wearing protective clothing, I may not visit these parts of the hospital after having been with my child: ▪Unit Kitchens and kitchen areas ▪Hospital School ▪ Play rooms ▪Family lounges or computer spaces The Nurse Station ____ If I opt out of wearing protective clothing, a daily change of clothes is required to help lessen spread of infection. ___ I know that, even if I opt out of wearing protective clothing, all other people who visit must follow the safety measures posted on the door. ____ I know that, in order to protect my child and others, if I have symptoms of an illness (e.g., cough, sore throat, fever, rash, diarrhea) I will not visit AFCH. If I do visit, I will tell the staff before my visit so that safety measures can be taken to avoid the spread of illness (e.g., wear a mask).
