Waiver: By signing this application, I agree to release the City of
San Buenaventura and Police Activities League (PAL), its agents, its
employees, and its certified volunteers from and against any claim
arising from my or my child’s participation in the program noted on
this Registration Form. I grant permission for the City of Ventura and
PAL to use participant’s image, filmed during program activities, to
promote its services and programs. I agree to indemnify and hold the
City of San Buenaventura and PAL harmless from and against any
claims, whether caused by passive negligence or otherwise. I will pay
all costs incident to any claim, including, without limitation, attorneys’ fees. I agree that this agreement is intended to be as broad and
inclusive as is permitted by law of the State of California.
Code of Conduct: I will abide by the rules – I will avoid arguments – I will
support everyone playing – I will play fair – I will follow directions. I will
be a role model of good sportsmanship: Winning without gloating – Losing
without complaining – Treating my opponents, coach and teammates with
respect and not criticizing. A player will receive a first warning for unsportsmanlike conduct. A player will receive a technical as a second warning for
unsportsmanlike conduct and must sit out ten minutes of that game. If a
player receives a second technical for unsportsmanlike conduct, he / she will
be suspended for the remainder of the game (but must stay on the bench
with his / her team). If a player receives a third technical for unsportsmanlike
conduct during the course of the season, he/she will be suspended from the
Westpark Basketball program for the remainder of the season.

Ciudad de Ventura y la Liga de Actividades de la Policia (PAL), sus agentes,
sus empleados y sus voluntarios certificados contra cualquier demanda

debe quedarse sentado durante diez minutos del juego. Si un/a jugador
recibe una segunda pena técnica de conducta antideportiva, él / ella será
suspendido por el resto del juego (pero debe permanecer en el banquillo
con su equipo). Si un/a jugador recibe una tercera pena técnica de conducta
antideportiva en el curso de la temporada, él / ella será suspendido de la
programa de fútbol de Westpark para el resto de la temporada.
