Dear Applicant: You are on the county’s list of voters who have submitted an Annual Application
for a Ballot by Mail (Annual ABBM). We have recently received information from the County
Voter Registrar that you have changed the residence address at which you are registered to vote, and
this address is different from the address on the Annual ABBM we had previously received from
you. In order for us to continue sending you ballots for any elections held in the remainder of this
calendar year, you must provide the Early Voting Clerk with an updated Annual ABBM. Please
complete the enclosed Annual ABBM and return to the Early Voting Clerk via mail, fax, or email.
We must receive the completed Annual ABBM by the following deadline in order to send you a
