¿Qué puede pasar si no tomo los fármacos a la hora indicada?
● Si dejas de tomar más de una dosis al mes, aumentará el riesgo de que tu
tratamiento deje de funcionar.
● Puede que aumente la cantidad de VIH en el cuerpo, lo que debilita el sistema
inmunitario.
● Es posible que alguno de los nuevos virus sea ‘resistente’ a los antirretrovirales.

websites and directories. Research and feedback suggests that these resources are valued by clinicians
and patients as providing relevant, accurate information. However, some patients find them too detailed,
complex or text-heavy.
Different people living with HIV have different information needs. Whereas some individuals are “information
seekers” who always try to keep up to date, others tend to rely on professionals to provide them with relevant
information and advice. They may only be ready to take on board new information at particular moments, for
example when there is a change in their medical circumstances (diagnosis, starting treatment, etc.).

However the qualitative research identified challenges in using analogies in printed resources. Whereas
some healthcare professionals regularly use analogies in discussion with patients, they are able to use their
judgement to select and adapt a metaphor so that it is appropriate for the individual patient. In print, an
analogy becomes fixed and unchanging, and may have unintended or negative connotations for some
individuals. We found that the analogy can become the focus of attention, distracting from rather than
aiding understanding of treatment issues.

Army has negative connotations for a person forced
to leave their country because of army persecution.
Mice are dirty and associated with poverty.
NAM aims to provide information for the widest range of people with HIV, but research has shown that we
particularly need to do more to reach black Africans with HIV living in the UK (40% of new diagnoses).
As a result, we decided to avoid the use of analogies and a more simple factual approach was taken
forward. A series of ten leaflets have been produced, entitled “The Basics”.
Feedback from healthcare professionals in the UK suggested that there was a need to develop a new
format for treatment education materials which would be appropriate for people who do not have much
prior understanding of HIV, whose first language is not English, who are less comfortable with written
information or who have lower literacy levels.
Each leaflet has been approved by the Plain English Campaign. The leaflets have been highly commended
at the British Medical Association Patient Information Awards, with the judges commenting: “Even when
there are complex issues to be dealt with, these leaflets have a great way of explaining things in a simple
and readable way… The use of graphics to help clarify the explanations is excellent… These are the best
leaflets I have reviewed over the years.”
Description
Our project was developed through close collaboration between a treatments writer, a graphic designer
and a group of health professionals who provide face-to-face treatments advice in HIV clinics across
London. The health professionals identified a need for a printed resource that could be used during a
discussion with a patient, rather than for solitary reading.
Together, a list of ten key topics to cover was developed (e.g. HIV and the immune system, CD4 and viral
load, starting treatment, side-effects, adherence, having a baby).
The health professionals told us that they often used illustrations and simple analogies to explain complex
health concepts. They shared their pictures and explanations, which the writer and designer then
developed into draft leaflets. Possible illustrations and explanations were refined over multiple sessions.
The draft leaflets were then tested in qualitative research conducted by an external research agency in
order to validate and refine the approach taken. Focus groups and in-depth interviews were conducted
with a diverse group of people with HIV as well as with professionals working both in clinical and
community settings.
Lessons learnt
We developed a new visual language in order to communicate key aspects of HIV infection and care.
Typically, a topic is explained in six images and less than 250 words. Leaflets using this approach are
designed to be used as part of a discussion between a healthcare professional and a patient.
NAM has an efficient and well-established ordering and distribution system for clinics and community
organisations which ensures that resources are made available to people with HIV across the UK. A total of
96,963 leaflets have been requested and distributed in one year (i.e. more than one leaflet for each person
with HIV in the UK). We estimate that over 80% of UK clinics have ordered copies.
Next steps
Although the project was planned for UK audiences, requests to translate or adapt the materials have
come from organisations in many other countries. In some cases, the leaflets would need to be adapted to
reflect local culture or clinical practice (especially in resource-limited settings).
Our current translation work concentrates on Europe, where variations in clinical practice are limited. We
work in partnership with HIV community organisations across the continent who review the text and
illustrations for local relevance and produce the translations.
Translations are already available in French, Spanish, Portuguese, Italian, German, Dutch, Polish, Russian,
Romanian and Turkish. In addition, in order to serve migrant communities in European countries,
translations are being produced in languages such as Mandarin, Arabic and Somali. Some of this work has
been initiated by gTt (Grupo de Trabajo sobre Tratamientos del VIH), our partner organisation in Spain.
